ABSTRACT

Objetivo: mapear los instrumentos para la evaluación del riesgo de lesiones por presión en adultos en situación crítica en una unidad de terapia intensiva; identificar los indicadores de desempeño de los instrumentos y la apreciación de los usuarios con respecto al uso/limitaciones de los instrumentos. Método: scoping review. Para redactar el estudio se utilizó la extensión Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews. La investigación se realizó mediante la herramienta de búsqueda EBSCOhost en 8 bases de datos, resultando 1846 estudios, de los cuales 22 conforman la muestra. Resultados: se identificaron dos grandes grupos de instrumentos: los generalistas [Braden, Braden (ALB), Emina, Norton-MI, RAPS y Waterlow]; y los específicos (CALCULATE, Cubbin & Jackson, EVARUCI, RAPS-ICU, Song & Choi, Suriaidi y Sanada y el índice COMHON). En cuanto al valor predictivo, EVARUCI y CALCULATE mostraron los mejores resultados de indicadores de desempeño. En cuanto a las apreciaciones/limitaciones señaladas por los usuarios, destaca la escala CALCULATE, seguida de la EVARUCI y la RAPS-ICU, aunque aún necesitan ajustes futuros. Conclusión: el mapeo mostró que las evidencias son suficientes para indicar uno o más instrumentos para la evaluación del riesgo de lesiones por presión en adultos críticos en una unidad de cuidados intensivos.


Objective: to map the instruments for risk assessment of pressure ulcers in adults in critical situation in intensive care units; identify performance indicators of the instrument, and the appreciation of users regarding the instruments' use/limitations. Method: a scoping review. We used the Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews in the writing of the study. We carried out the searches in the EBSCOhost search tool for 8 databases, resulting in 1846 studies, of which 22 studies compose the sample. Results: we identified two big instrument groups: generalist [Braden, Braden (ALB), Emina, Norton-MI, RAPS, and Waterlow]; and specific (CALCULATE, Cubbin & Jackson, EVARUCI, RAPS-ICU, Song & Choi, Suriaidi and Sanada, and COMHON index). Regarding the predictive value, EVARUCI and CALCULATE presented better results for performance indicators. Concerning appreciation/limitations indicated by users, we highlight the CALCULATE scale, followed by EVARUCI and RAPS-ICU, although they still need future adjustments. Conclusion: the mapping of the literature showed that the evidence is sufficient to indicate one or more instruments for the risk assessment of pressure ulcers for adults in critical situation in intensive care units.

Método: scoping review. O Preferred Reporting Items for Systematic Reviews and Meta-Analyses Extension for Scoping Reviews foi utilizado para a redação do estudo. A pesquisa foi realizada na ferramenta de busca EBSCOhost em oito bases de dados, resultando em 1846 estudos, dos quais 22 compõem a amostra. Resultados: identificaram-se dois grandes grupos de instrumentos: os genéricos [Braden, Braden (ALB), Emina, Norton-MI, RAPS e Waterlow]; e os específicos (CALCULATE, Cubbin & Jackson, EVARUCI, RAPS-ICU, Song & Choi, Suriaidi e Sanada e o índice de COMHON). Quanto ao valor preditivo, a EVARUCI e a CALCULATE apresentaram os melhores resultados de indicadores de desempenho. Em relação à apreciação/às limitações apontadas pelos utilizadores, destacam-se a escala CALCULATE, seguindo-se da EVARUCI e da RAPS-ICU, embora ainda necessitem de ajustes futuros. Conclusão: o mapeamento mostrou que as evidências são suficientes para indicar um ou mais instrumentos para avaliação do risco de lesões por pressão nos adultos em situação crítica em unidade de cuidados intensivos.

ABSTRACT

Introducción: Los extractos alergénicos Valergen® (Dermatophagoides pteronyssinus, Blomia tropicalis y Dermatophagoides siboney), desarrollados en el Centro Nacional de Biopreparados para usarlos en pruebas cutáneas de pacientes con alergia, han demostrado altas sensibilidad y especificidad. Objetivo: Determinar la sensibilidad ante 2 concentraciones diferentes de extractos alergénicos de ácaros mediante pruebas cutáneas por punción. Métodos: Se realizó un estudio observacional, descriptivo y transversal de 230 pacientes de 5 a 49 años de edad, atendidos en el Policlínico Docente Ernesto Guevara de la Serna de Niquero, provincia de Granma, durante el 2022. Las variables analizadas fueron respuestas positivas según la edad, tipo de extractos alergénicos, sensibilización según enfermedades alérgicas, así como reacción adversa local. Resultados: En ambas concentraciones, la de 20 000 UB/ml y la de 2000 UB/ml, predominaron el grupo etario de 5 a 13 años (47,0 %), las respuestas positivas (27,8 y 25,7 % para cada dilución) y el extracto alergénico Dermatophagoides pteronyssinus (con 93,9 y 89,1 %, respectivamente). La rinitis fue la enfermedad alérgica con mayor sensibilización (29,1 % en la primera concentración y 28,3 % en la segunda). Los efectos adversos locales se presentaron solo en la de 20 000 UB/ml (3,9 %), con un diámetro promedio del habón de 15,9 mm (IC 95 %: 15,4 - 16,4 mm) para la reacción superior. Conclusiones: Los extractos alergénicos de ácaros con una concentración de 2000 UB/ml resultaron igualmente sensibles y seguros que la dilución establecida de 20 000 UB/ml, la cual es reconocida a escala nacional por su calidad como medio diagnóstico.


Introduction: The Valergen® allergenic extracts (Dermatophagoides pteronyssinus, Blomia tropicalis and Dermatophagoides siboney), developed in the National Center for Biopreparations to use them in cutaneous tests to the patients with allergy, have demonstrated a high sensibility and specificity. Objective: To determine the sensibility considering 2 different concentrations of mite allergenic extracts by means of needle cutaneous tests. Methods: An observational, descriptive and cross-sectional study of 230 patients aged 5 to 49 was carried out. They were assisted in Ernesto Guevara de la Serna Teaching Polyclinic in Niquero, Granma province, during 2022. The analyzed variables were positive responses according to age, type of allergenic extracts, sensitization according to allergic diseases, as well as local adverse reaction. Results: In both concentrations, 20 000 UB/ml and 2000 UB/ml, there was a prevalence of the 5 to 13 age group (47.0%), the positive responses (27.8 and 25.7% for each dilution) and the Dermatophagoides pteronyssinus allergenic extract (93.9 and 89.1%, respectively). Rhinitis was the allergic disease with more sensitization (29.1 in the first concentration and 28.3% in the second one). The local adverse events were just presented in that of 20 000 UB/ml (3.9%) with an average wheal diameter of 15.9 mm (IC 95%: 15.4 - 16.4 mm) for the higher reaction. Conclusions: Mite allergenic extracts with a concentration of 2000 UB/ml were equally sensitive and safe than the established dilution of 20 000 UB/ml, which is recognized on a national scale by its quality as diagnosis method.

ABSTRACT

Evaluación del Índice Peso-Circunferencia de Cintura (IPCC) mediante Regresión Logística. Estudio descriptivo, prospectivo y transversal, muestra 1095 adultos y adultos mayores, edad entre 20 y 96 años. Métodos: variables: edad, peso, talla, Circunferencia de Cintura (CC), IMC, Índice Cintura-Talla (ICT). Resultados: IMC similar en los dos sexos; promedios de CC e ICT mayor en grupo ≥ 65 años; IPCC mayor en grupo 20-59 años; porcentaje de riesgo del IPCC(54,1%) mayor en CC (44,7%) menor en ICT (78,2%). IPCC significativamente asociado al IMC, CC e ICT (p<0,000); valor predictivo positivo 0,92 y valor predictivo negativo 0,70, indican capacidad discriminativa; Riesgo Relativo, 1,92; el estadístico de prueba Wald indica significancia estadística para los coeficientes de ecuaciones de probabilidad de riesgo; área bajo la curva ROC es 0,803 y 0,903 (P<0,000), alta sensibilidad y especificidad. Conclusión: el IPCC puede considerarse para evaluar sobrepeso y obesidad en adultos, dada su alta capacidad discriminativa(AU)


Evaluation of the Waist Weight-CircumferenceIndex (WWCI) using Logistic Regression. Descriptive, prospective and cross-sectional study shows 1095 adults andolder adults, aged between 20 and 96 years. Methods: variables: age, weight, size, Waist Circumference (WC), BMI, Waist-SizeIndex (WSI). Results: Similar BMI in both sexes; higher WCand WSI averages in a ≥ age of 65; major WWCI in group20-59 years; WWCI risk rate (54.1%) higher in WC (44.7%)WSI (78.2%). WWSI significantly associated with BMI, WCand WSI (p<0.000); positive predictive value 0.92 and negative predictive value 0.70, indicate discriminatory capacity; RelativeRisk, 1.92; Wald test statistic indicates statistical significance for the coefficients of risk probability equations; area under theROC curve is 0.803 and 0.903 (P<0.000), high sensitivity and specificity. Conclusion: WWCI can be considered to evaluate overweight and obesity in adults, given its high discriminatory capacity(AU)

El autor describe el trabajo de supervisión como de escuchar la escucha del analista frente a su analizando. L'autrice décrit le travail de supervision en tant que l'ecoute de l'écoute de l'analyste face à son analysant. Elle suggère que cette Écoute de supervision idéalement agit dans un espace semblable à une atmosphère presque onirique. La chercheuse mentionne ensuite une rêverie d'une autre rêverie, comme une partie d'un processus de déplacement du sens commun et elle discute du rôle de la théorie analytique pendant la séance et dans une supervision. La présence de la théorie est comparée à présence du sang dans le corps humain : elle ne doit jamais peser sur la relation intersubjective du champ analytique.

ABSTRACT

El objetivo es evaluar el IPCC, para proponerlo como indicador de sobrepeso y obesidad. Es un estudio descriptivo, prospectivo, transversal y correlacional, en una muestra de 896niños y adolescentes con edad de 6 y 19 años; 452 (50,4%)del sexo masculino y 444 (49,6%) del sexo femenino. Métodos: Se midieron las variables sexo, edad, peso, talla, IMC, PGC e IPCC; se determinaron promedios, desviación, correlaciones y curva ROC. Resultados: promedios de peso 43,1±14,8 kg; talla 146,2±15,8 cm; IMC 19,6±3,6 kg/m2; CC 66,7±9,3 cm; IPCC 0,64±0,2 y PGC 15,5±7,3. Correlaciones muy buenas entre Talla-Peso (0,87); Peso-IMC (0,84); Peso-IPCC (0,91);Talla-IPCC (0,80) e IMC-IPCC (0,75); porcentajes resultantes de clasificar a niños y adolescentes, según valores del IMC eI PCC, y los tres criterios asumidos como referencias, indican que: Sobrepeso 10,9% (IMC) y 11,6% (IPCC); Obesidad 5,3%(IMC) y 5,8% (IPCC). Para el criterio Media y Desviación, Sobrepeso 4,1% (IMC) y 8,1% (IPCC); Obesidad 0,3% (IMC)y 4,2% (IPCC). Para el criterio Fundacredesa, Sobrepeso 7,2%(IMC) y 8,0% (IPCC); Obesidad 3,3% (IMC) y 3,6% (IPCC).Promedios, por sexo, para las variables peso, talla, IMC, CC eI PCC, no significativos; si los promedios del PGC (p<0,000).Curva ROC se aleja de la diagonal, aunado al hecho que el área bajo la curva es 0,983, refleja la bondad del modelo, complementado con una alta sensibilidad y especificidad, garantizado así un al poder discriminante. The objective is to evaluate the WCWI, to propose it as an indicator of overweight and obesity. It is a descriptive, prospective, cross-cutting and correlational study, in a sample of 896 children and adolescents aged 6 and 19; 452 (50.4%)444 (49.6%) of the female sex. ABSTRACT

Introduction: The SARS-CoV-2 virus is a positive-strand RNA virus. The virus can also be detected in many different specimens as throat swabs, nasal swabs, sputum, saliva, blood, etc. Objective: The aim of this paper is to compare the reliability of different types of specimen collection, saliva and swabs samples for the detection of SARS-CoV-2. Material and Methods: A sample of 22 COVID-19 positive patients was selected. Paired samples from saliva, nasopharyngeal, oropharyngeal and nasopharyngeal + oropharyngeal swabs were collected on the 7th day after diagnosis. The hyssops and medium employed was IMPROSWAB and IMPROVIRAL NAT Medium, Germany. The sample evaluation was conducted through RT-PCR. The results were compared using Fisher's exact test and ROC curve. The gold standard proposed in this paper was the nasopharyngeal + oropharyngeal swabs specimen. Results: The gold standard method detected 10 true positive cases, of which oropharyngeal swabs, nasopharyngeal swabs and saliva only detected three positive cases. Significant differences (Fisher's exact test p = 0.003) were detected in the comparison between saliva and the gold standart proposed. The ROC curve analysis showed that saliva had an area under the curve of 0.650, with a 30 percent of sensibility. However, the nasopharyngeal and nasopharyngeal + oropharyngeal samples had an area under curve of 0.950 and 1.000, respectively, with a sensibility of 90 percent and 100 percent, respectively. Conclusion: Saliva samples are not a reliable specimen for SARS-CoV-2 RNA detection. In turn, the most reliable specimens are nasopharyngeal and nasopharyngeal + oropharyngeal samples collected by swabbing(AU)


Introducción: El SARS-CoV-2 es un virus ARN positivo. Este virus puede ser detectado en diferentes tipos de secreción como hisopada bucal, nasal, esputo, saliva, sangre, etc. Objetivo: El objetivo de este estudio es comparar la confiabilidad de diferentes tipos de muestras, saliva y exudado, en la detección de SARS-CoV-2. Material y Métodos: Una muestra de 22 pacientes con diagnóstico de Covid-19 fue estudiada. Se tomaron muestras pareadas de saliva y exudado nasofaríngeo y orofaríngeo en cada paciente. Se emplearon los hisopos y medios de la firma alemana IMPROVE®. Los resultados de las determinaciones por RT-PCR se compararon mediante test de Fisher (test de la probabilidad exacta de Fisher) y cada sets de muestras fue evaluada individualmente y luego comparadas por curvas ROC. El estándar de oro propuesto fue el doble hisopado nasofaríngeo/orofaríngeo. Resultados: El método de oro propuesto detectó 10 casos positivos. La coincidencia de detección entre todos los sets de muestras fue de 3 casos (30 por ciento). Se obtuvieron diferencias significativas (Fisher p = 0.003) en la comparación de los casos detectados en saliva vs el estándar de oro. El análisis de curvas ROC mostró un área bajo la curva de 0.650 (30 por ciento de sensibilidad) para la saliva. En el caso del hisopado nasofaríngeo y el estándar de oro mostraron un área bajo la curva de 0.95 y 1.00, respectivamente, con una sensibilidad del 90 (AU) por ciento y 100 por ciento, respectivamente. Conclusiones: La saliva no es una muestra confiable para la detección de SARS-CoV-2. La muestra más confiable para el diagnóstico fue el hisopado nasofaríngeo y el doble hisopado(AU)

Objetivo: O presente estudo tem como objetivo avaliar o perfil de sensibilidade antimicrobiana do patógeno mais comum causador da infecção do trato urinário (ITU) de gestantes que foram internadas em um hospital de ensino do município de São Paulo em determinado período. Métodos: Estudo retrospectivo, transversal, quantitativo, realizado avaliando as uroculturas positivas e o perfil de sensibilidade antimicrobiana dos agentes mais comuns encontrados em ITUs das gestantes de hospital e maternidade-escola do município de São Paulo de janeiro de 2019 até janeiro de 2020. Resultados: A partir da análise de uroculturas positivas e antibiograma de 149 gestantes admitidas com quadro de infecção urinária no referido hospital no intervalo de tempo analisado, constatou-se que 83,89% dos casos apresentaram como patógeno a bactéria Escherichia coli. No âmbito da resistência bacteriana, percebeu-se que o maior índice foi encontrado no que tange a cefalotina (65%), ampicilina (58%) e ampicilina/sulbactam (45%). Ademais, a partir das análises individuais, 20 pacientes, ou seja, aproximadamente 13,42% apresentaram cepas sensíveis a todas as medicações apontadas, e as demais apresentaram resistência a, pelo menos, uma delas. Conclusão: A partir da premissa de eficácia desempenhada pelo protocolo de medicação empírica estabelecido pela instituição no tocante ao tratamento de infecção do trato urinário em gestantes, a cefalotina certamente não deveria compor o rol de drogas ofertadas às pacientes. Isso se dá, pois a sensibilidade apresentada pela Escherichia coli, patógeno que mais comumente está associado aos quadros de ITU do serviço, a essa droga é muito baixa. Já a nitrofurantoína apresentou um satisfatório espectro de cobertura, sendo a resistência à droga inferior a 10%. Com isso, conclui-se que ela deve permanecer como droga inicial para as ITUs das gestantes que chegam a essa instituição.(AU)


Objective: The present study aims to evaluate the antimicrobial sensitivity profile of the most common pathogen that causes urinary tract infection (ITU) in pregnant women who were admitted to a Teaching Hospital in the city of São Paulo in a specific period. Methods: Retrospective, cross-sectional, quantitative study carried out evaluating positive urine cultures and the antimicrobial sensitivity profile of the most common agents found in ITU of pregnant women at Teaching Maternity hospital in the city of São Paulo from January 2019 to January 2020. Results: From the of positive urine culture and antibiogram of 149 pregnant women admitted with a urinary tract infection in the referred hospital in the analyzed period of time, it was found that 83.89% of the cases presented the bacterium Escherichia coli as a pathogen. In the scope of bacterial resistance, it was noticed that the highest index was found with respect to Cephalothin (65%), ampicillin (58%) and ampicillin/sulbactam (45%). Furthermore, from the individual analyzes, 20 patients, that is, approximately 13.42% had strains sensitive to all the medications indicated, with the others showing resistance to at least one of them. Conclusion: Based on the premise of efficacy performed by the empirical medication protocol established by the institution regarding the treatment of urinary tract infection in pregnant women, Cephalothin should certainly not be included in the list of drugs offered to patients. This happens because the sensitivity presented by Escherichia coli, the most commonly pathogen associated with the UTI pathogen of the service, to this drug is very low. Nitrofurantoin, on the other hand, presented a satisfactory coverage spectrum, with drug resistance below 10%. Thus, it is concluded that this should remain as an initial drug for ITUs of pregnant women who arrive at this institution.(AU)

RESUMEN El objetivo de este estudio fue determinar la actividad antimicrobiana de un cultivo de Streptomyces sp. 6E3 aislado de minerales frente a diferentes cepas patógenas, producir un extracto y estimar la concen tración mínima inhibitoria (CMI) de las fracciones contra Staphylococcus aureus resistente a meticilina (SARM). La cepa Streptomyces sp. 6E3 mostró actividad antimicrobiana principalmente contra Staphy lococcus aureus (S. aureus). Cinco de las seis fracciones presentaron actividad antimicrobiana y la más efectiva dio una CMI de 0,88 ug/mL frente a S. aureus ATCC 33862, 0,44 ug/mL frente a S. aureus ATCC 43300 y 1,76 ug/mL frente a S. aureus cepa SARM. Streptomyces sp. 6E3 tiene un potencial antimicrobiano frente a cepas de S. aureus resistentes a meticilina y no resistentes, siendo de interés la realización de más estudios sobre sus metabolitos activos.


ABSTRACT The objectives of this study were to determine the antimicrobial activity of a culture of Streptomyces sp. 6E3 isolated from minerals against different pathogenic strains, to produce an extract and to estimate the minimum inhibitory concentration (MIC) of the fractions against methicillin-resistant Staphylococ cus aureus (MRSA). Streptomyces sp. 6E3 showed antimicrobial activity primarily against Staphylococcus aureus (S. aureus). Five of the six fractions presented antimicrobial activity and the most effective gave a MIC of 0.88 ug / mL against S. aureus ATCC 33862, 0.44 ug / mL against S. aureus ATCC 43300 and 1.76 ug / mL vs. a S. aureus MRSA strain. Streptomyces sp. 6E3 has an antimicrobial potential against S. aureus strains resistant to methicillin and non-resistant, being of interest carrying out of more studies on its active metabolites.

ABSTRACT

Apresento a escrita de minha experiência de criação do campo de estágio em Plantão Psicológico fundamentado na Abordagem Gestáltica num curso de graduação em Psicologia numa Instituição particular de Ensino Superior no Rio de Janeiro. Sendo escassa a literatura gestáltica neste tema, recorri a autores baseados em Carl Rogers, em cuja abordagem há farta oferta de publicações, o que me permitiu criar boas conexões com autores da Gestalt-terapia. Chamei para compor esse diálogo autores da Psicologia Social e, assim, proponho o único atendimento no Plantão Psicológico Gestáltico como uma prática de clínica ampliada que pró-move deslocamentos nos fazeres acadêmicos na graduação em Psicologia por expandir as práticas clínicos na clínica-escola para além da psicoterapia bipessoal e por propor a sensibilidade como condição de encorpamento ético nos fazeres psicológicos. Finalizo, indicando o Plantão Psicológico Gestáltico como uma prática política e inclusiva, portanto, democrática, contemplando contemporaneamente a demanda por atenção psicológica pontual, em diferentes espaços e contextos - fomentando atenção especialmente à nossa formação universitária. I present the writing of my experience of creating the field of internship on Psychological Duty based on the Gestaltic Approach in a graduation course in Psychology in a private Institution of Higher Education in Rio de Janeiro. Being the gestalt literature scarce on this subject, I have resorted to authors based on Carl Rogers, in whose approach there is a large supply of publications, which allowed me to create good connections with Gestalt-therapy authors. I called authors of Social Psychology to compose this dialogue and so, I propose the only service in the Gestaltic Psychological Duty as an expanded clinic practice, which promotes displacements in the academic studies in Psychology by expanding clinical practices in the clinic-school beyond bipersonal psychotherapy and for proposing sensitivity as a condition of ethical embodiment in psychological studies. I conclude by indicating the Gestaltic Psychological Duty as a political and inclusive practice, therefore, democratic, contemporaneously contemplating the demand for punctual psychological attention, in different spaces and contexts - fostering attention especially to our university education. Presento mi experiencia en la creación de la pasantía sobre el Deber Psicológico basada en el Enfoque Gestáltico en un curso de graduación en Psicología en una Institución de Educación Superior privada en Rio de Janeiro. Como hay poca literatura gestáltica sobre este tema, he recurrido a autores basados en Carl Rogers, en cuyo enfoque hay una gran oferta de publicaciones, lo que me permitió crear buenas conexiones con los autores de la terapia Gestalt. Llamé a los autores de la Psicología Social para componer este diálogo y así, propongo el único servicio en el Deber Psicológico Gestáltico como una práctica clínica expandida, que promueva desplazamientos en los estudios académicos de la Psicología para expandir las prácticas clínicas en la clínica-escuela más allá de la psicoterapia bipersonal y para proponer la sensibilidad como condición de incorporación ético en los estudios psicológicos. Concluyo señalando el Deber Psicológico Gestáltico como una práctica política e inclusiva, así que democrática, que contempla la demanda de atención psicológica puntual, en diferentes espacios y contextos - fomentando la atención especialmente a nuestra educación universitaria. ABSTRACT

Se ha demostrado la asociación de los microbios intestinales Firmicutes y Bacteroidetes con la hipertensión arterial, inclusive en personas sensibles a la sal. Asimismo, la disminución de la microbiota por los antibióticos puede aumentar o disminuir la tensión arterial acorde a la influencia del genotipo. En este artículo se comunican algunos argumentos sobre el tema y, además, se explica cómo la función biológica de los probióticos también puede ser el resultado de modificaciones epigenéticas, relacionado, en parte, con el micro-ARN, y cómo los productos de la fermentación de los nutrientes por la microbiota pueden influir en la tensión arterial regulando el gasto energético, el metabolismo intestinal de las catecolaminas, el transporte iónico gastrointestinal y renal y, con ello, la sensibilidad a la sal. Por último, se concluye que el efecto beneficioso o deletéreo de la microbiota intestinal sobre la tensión arterial es una consecuencia de distintas variables, incluidas las genéticas y epigenéticas, así como el estilo de vida y el uso de antibióticos.


The association of Firmicutes and Bacteroidetes intestinal microbes with hypertension has been demonstrated, including people sensitive to salt. Also, the microbiota decrease due to antibiotics can increase or diminish the blood pressure according to the genotype influence. Some arguments on the topic are communicated in this work and, also, it is explained how the biological function of probiotics can also be the result of epigenetics modifications, related, to some extent, to the micro-RNA, and how the nutrients fermentation products due to microbiota can influence in blood pressure regulating the energy expense, intestinal metabolism of catecholamines and gastrointestinal and renal ionic transportation, and with it the sensibility to salt. Lastly, it is concluded that the beneficial or deleterious effect of intestinal microbiota on blood pressure is a consequence of different variables, including genetics and epigenetics variables, as well as lifestyle and use of antibiotics.

ABSTRACT

Este estudo teve como objetivo central investigar como o brincar e o se-movimentar são proporcionados no espaço escolar e de que maneira eles contribuem para a educação da sensibilidade da criança pequena. Foi desenvolvido um estudo de caso que envolveu duas escolas municipais da cidade de Capinzal, SC; uma localizada no interior do município e a outra na cidade. Foi selecionada uma turma de crianças entre 4 e 6 anos de idade em cada escola, que totalizaram 40 crianças. Constatou-se que o brincar e o se-movimentar nas escolas estudadas acontecem muitas vezes por intermédio do controle dos professores, sempre com o intuito de promover uma competição e de chegar a um resultado final. Portanto, nas escolas investigadas, o incentivo à construção de situações que possibilitem o desenvolvimento da sensibilidade recebe pouca importância, o que compromete significativamente a educação da sensibilidade dessas crianças e limita o poder de criação.


Thus, this study had as main objective to investigate about the ways as the play and the move yourself are proposed in the school environment and how they contribute for the sensibility education to the child. First, it was developed a theorical research about the subjets and the fundamental authors, that suply the base for the second part of the study. This other step was an empirical study that involved two municipal schools from the city of Capinzal, SC; one located in the countryside and the other one downtown. It was chosen a class of chuildren from 4 to 6 years old in each school, that totaling 40 children. Therefore, in the investigated schools, the stimulus to situations construction that enable the sensibility development receives little importance, what commits meaningly the sensibility education of these children and limits the creation' power.


Este estudio tuvo como objetivo central investigar cómo el juego y el movimiento se proporcionan en el espacio escolar y de qué manera contribuyen a la educación de la sensibilidad del niño pequeño. Se desarrolló un estudio de caso que involucró dos escuelas municipales de la ciudad de Capinzal, SC; una ubicada en el interior del municipio y la otra en la ciudad. Se seleccionó una clase de niños entre 4 y 6 años de edad en cada escuela, que totalizaron 40 niños. Se constató que el jugar y el mudanza en las escuelas estudiadas ocurren muchas veces por intermedio del control de los profesores, siempre con el propósito de promover una competición y de llegar a un resultado final. Por lo tanto, en las escuelas investigadas, el incentivo a la construcción de situaciones que posibiliten el desarrollo de la sensibilidad recibe poca importancia, lo que compromete significativamente la educación de la sensibilidad de esos niños y limita el poder de creación.

ABSTRACT

Abstract The present study assessed oxygen saturation (SaO2) levels before, during, and after at-home bleaching treatment in the pulps of healthy maxillary central incisors. SaO2 levels were measured in 136 healthy maxillary central incisors using a pulse oximeter. The bleaching protocol consisted of 10% carbamide peroxide gel placed in individual trays and used for four hours daily for 14 days. SaO2 levels were assessed before bleaching (T0), immediately after the first session (T1), on the 7th day of treatment (T2), on the 15th day (the day following the last session) (T3), and 30 days after completion of the bleaching protocol (T4). Data were statistically analyzed using generalized estimating equations (GEE), Student's t test (p<0.05) and Pearson's correlation. Mean pulp SaO2 levels were 85.1% at T0, 84.9% at T1, 84.7% at T2, 84.3% at T3, and 85.0% at T4. Gradual reductions in SaO2 levels were observed, with significant differences (p<0.001) during the course of home bleaching treatment. However, 30 days after the end of the bleaching protocol, SaO2 levels returned to baseline levels. Home bleaching caused a reversible transient decrease in SaO2 levels in the pulps.


Resumo Este estudo verificou o grau de saturação de oxigênio (SaO2) pulpar antes, durante e após o clareamento dental caseiro em incisivos centrais superiores hígidos. O nível de SaO2 foi verificado em 136 incisivos centrais superiores hígidos usando oxímetro de pulso. A técnica de clareamento empregou peróxido de carbamida 10% em moldeira individual por quatro horas diárias durante 14 dias. Os níveis de SaO2 foram analisados antes do clareamento (T0), imediatamente após a primeira sessão (T1), no sétimo dia de tratamento (T2), no décimo quinto dia (um dia após a última sessão) (T3) e 30 dias após o término do clareamento dental (T4). A análise estatística utilizou o modelo de equações de estimações generalizadas (GEE), teste t de Student (p<0,05) e correlação de Pearson. Os níveis médios de SaO2 pulpar foram 85,1% em T0, 84,9% em T1, 84,7% em T2, 84,3% em T3 e 85,0% em T4. Foi observada uma redução gradual dos níveis de SaO2, com diferenças significantes (p<0,001) durante o clareamento dental caseiro. No entanto, 30 dias após o término do clareamento dental, houve retorno aos valores iniciais. O clareamento dental caseiro provocou uma diminuição transitória reversível no grau de SaO2 pulpar.

Introducción: La sensibilidad, especificidad, valor predictivo, índice de validez y razón de verosimilitud, son comúnmente empleados como indicadores del desempeño de las pruebas diagnósticas en el ámbito médico, pero ¿son suficientes para concluir que un test es aceptable para la práctica clínica?. Objetivo: Evaluar la utilidad de cada uno de estos indicadores y sus limitaciones, basándose en el análisis de un caso concreto. Métodos: Se desarrolló un estudio bibliográfico para identificar los usos y abusos de los indicadores del desempeño de pruebas diagnósticas que con mayor frecuencia se emplean en la literatura. Conclusiones: Los resultados indican que las herramientas estadísticas son un apoyo a la toma de decisiones sobre la calidad de un test diagnóstico cuando se interpretan adecuadamente dentro de un marco conceptual y metodológico viable, reproducible y aplicable a la población diana que se desea evaluar(AU)


Introduction: Sensitivity, specificity, predictive value and validity index and likelihood ratio are commonly used as indicators of the performance of diagnostic tests, but Are they sufficient to conclude that a test is acceptable for clinical practice?. Objective: To evaluate the usefulness of each one of these indicators and their limitations, based on the analysis of a recent report. Methods: A bibliographic study was developed to identify the uses and abuses of the performance indicators of diagnostic tests that are most frequently used in the literature. Conclusion: The results indicate that the statistical tools are a support for making decision on the quality of a diagnostic test but they must be interpreted properly within a viable conceptual and methodological framework, reliable and applicable to the target population to be evaluated(AU)

Mycobacterium tuberculosis (M. tuberculosis) es un microorganismo cuya importancia como agente infeccioso ha permanecido a través de los años, pero que se ha convertido en una emergencia y un grave problema de salud pública, como respuesta a la evolución en su comportamiento ante los antimicrobianos de primera línea para su tratamiento y al surgimiento de cepas multi-resistentes, lo que amerita el uso de alternativas terapéuticas que permitan su control. El objetivo del trabajo fue evaluar el comportamiento in vitro de M. tuberculosis ante el antimicótico fluconazol, para su posible uso como alternativa terapéutica. Para ello, se evaluaron 6 cepas de M. tuberculosis: 2 resistentes a rifampicina, 2 resistentes a isoniazida y 2 sensibles a ambos antimicrobianos, Se utilizó el método de Concentración Inhibitoria Mínima, utilizando la técnica en microplaca con Azul de Alamar y la técnica de dilución en tubo. Ambas metodologías mostraron sensibilidad ante bajas concentraciones de fluconazol (0,0625 µg/ml). Todas las cepas fueron sensibles a la combinación fluconazol/isoniazida; mientras que, a la combinación fluconazol/rifampicina mostraron resistencia, indicando el efecto antagónico de la rifampicina sobre el fluconazol. Los resultados permiten concluir y sugerir el posible uso terapéutico del fluconazol ante las infecciones asociadas por M. tuberculosis.


Mycobacterium tuberculosis (M. tuberculosis) is a microorganism whose importance as an infectious agent has remained over the years but which has become a recent emergency and a serious public health problem in response to the evolution in its behavior against first-line antimicrobials, for its treatment and the emergence of multi-resistant strains, which require the use of therapeutic alternatives that allow its control. The objective of the work was to evaluate the in vitro behavior of M. tuberculosis before the antifungal agent fluconazole, for its possible use as a therapeutic alternative. To this, six strains were evaluated M. tuberculosis: 2 resistants to rifampicin, 2 resistants to isoniazid and 2 sensitive to both antimicrobials. We used the method of Minimum Inhibitory Concentration, using the microplate technique with Alamar Blue and the tube technique. Both methodologies showed sensitivity to low concentrations of fluconazole (0.0625 µg/ml). All strains were sensitive to the fluconazole / isoniazid combination; whereas, when exposed to the fluconazole / rifampicin combination, the strains showed resistance, indicating the antagonistic effect of rifampicin on fluconazole. The results allow us to conclude and suggest the possible therapeutic use of fluconazole against infections associated with M. tuberculosis.

Abstract This study determined the oxygen saturation (SaO2) in dental pulp of healthy maxillary and mandibular molars. Mean of SaO2 was evaluated in 112 maxillary and mandibular molars using pulse oximetry. Quantitative variables were described by mean and standard deviation. Variables with symmetric distribution were compared by Student t test and Mann-Whitney test. Pearson's correlation coefficient was used to correlate quantitative variables. Analysis of variance was used to assess differences in SaO2 levels between the molar groups, followed by post-hoc Tukey. The significance level established at p<0.05. Mean of oxygen saturation for the 112 molar dental pulps was 85.09%. There was no significant correlation (r=-0.007; p=0.977) between the mean of SaO2 of molar pulps with patient´s indicator finger (92.89%). There was a significant difference (p=0.037) between the mean of SaO2 of the first (85.76%) and second maxillary molars (81.87%), and it was not significant (p=0.1775) between the first and second mandibular molars. Maxillary molars had lower pulpal SaO2 (83.59%) than mandibular molars (86.89%) (p=0.018). The mean of the patient's response time to the cold stimulus was 1.12 s (maxillary molars 1.25 s and mandibular molars 0.99 s)(p=0.052). There was no significant correlation between the time response of the patient to the cold stimulus and the SaO2 for molars. The mean oxygen saturation level was 85.09%. The mandibular molars presented higher SaO2 level than maxillary molars.


Resumo Este estudo determinou o nível de saturação de oxigênio (SaO2) em polpas dentais hígidas de molares. O nível de SaO2 foi avaliado em 112 molares superiores e inferiores usando oxímetro de pulso. As variáveis quantitativas foram descritas pela média e desvio padrão. As variáveis com distribuição simétrica foram comparadas pelo teste t de Student e teste de Mann-Whitney. O coeficiente de correlação de Pearson foi utilizado para correlacionar variáveis quantitativas. A análise de variância foi utilizada para avaliar as diferenças nos níveis de SaO2 entre os grupos de molares, seguido de Tukey pós-hoc. A significância foi estabelecida em 0,05. O nível médio de SaO2 para as polpas de 112 molares foi de 85,09%, não havendo correlação com a média de SaO2 do dedo indicador do paciente (92,89%). Houve diferença significativa entre o nível médio de SaO2 dos primeiros molares superiores (85,76%) e os segundos molares superiores (81,87%) e não foi significativo entre os primeiros e os segundos molares inferiores. Os molares superiores apresentaram menor nível de SaO2 (83,59%) do que os molares inferiores (86,89%). A média do tempo de resposta do paciente ao estímulo com frio foi de 1,12 s (molares superiores 1,25 segundos e molares inferiores 0,99 segundos). Não houve correlação significativa entre o tempo de resposta do paciente ao estímulo com frio e o nível de saturação de oxigênio para os molares. Em resumo, o nível médio de saturação de oxigênio foi de 85,09%. Os molares inferiores apresentaram maior nível de SaO2 do que os molares superiors

Abstract The aim of this study was to determine oxygen saturation levels in the dental pulp of maxillary premolars in different age groups. A total of 120 human maxillary premolars with normal dental pulps were selected covering the following age groups: 20-24, 25-29, 30-34, 35-39 and 40-44 years (n=24 each group). Oxygen saturation was assessed using pulse oximetry. Analysis of variance was used to assess differences in oxygen saturation levels and Tukey's test was used to identify the age groups that differed from each other. Significance was set at 0.05. Mean oxygen saturation of 120 premolars was 86.20% considering all age groups. Significantly reduced levels were found in the oldest group compared to the other groups: 40 to 44 years - 80.00% vs. 89.71, 87.67, 88.71, and 84.80% for age groups 20-24, 25-29, 30-34, 35-39 years, respectively. The mean oxygen saturation levels were similar between 20 and 39 years of age (86.20%) in the whole sample, but reduced significantly in the 40-44-year age group, suggesting that older patients present lower oxygen saturation results even in the absence of pulp tissue injury.


Resumo Este estudo determinou os níveis de saturação de oxigênio (SaO2) em polpas dentárias de pré-molares superiores em diferentes faixas etárias. Foram selecionados 120 pré-molares superiores humanos com polpas dentárias normais, abrangendo os seguintes grupos etários: 20-24, 25-29, 30-34, 35-39 e 40-44 anos (n=24 para cada grupo). A saturação de oxigênio foi avaliada utilizando oximetria de pulso. A análise de variância foi utilizada para avaliar diferenças nos níveis de saturação de oxigênio, e o teste de Tukey foi utilizado para identificar os grupos etários que diferiam uns dos outros. A significância foi estabelecida em 0,05. A saturação média de oxigênio foi de 86,20% considerando todos os grupos etários. Níveis significativamente reduzidos foram encontrados no grupo de indivíduos de maior idade em comparação aos outros grupos: 40 a 44 anos - 80,00% vs. 89,71, 87,67, 88,71 e 84,80% para os grupos etários 20-24, 25-29, 30-34, 35-39 anos. Os níveis médios de saturação de oxigênio foram semelhantes entre os 20 e os 39 anos de idade (86,20%), mas reduziram-se significativamente na faixa etária de 40-44 anos, sugerindo que os pacientes mais idosos apresentam menor saturação de oxigênio mesmo na ausência de lesão do tecido pulpar.

Objective To observe the expression of signal transducer and activator of transcription (STAT3) in chronic myeloid leukemia cells K562 and analyze the influence of these changes to the cell proliferation,differentiation and drug resistance.Methods We constructed recombinant eukaryotic expression vector siRNA-STAT3,and transfected them into K562 cells with the help of lipofactamine 3000.Then stable monoclonal cells would be screened by puromycin and validated by Western blot.Inhibition rate of the cell growth to imatinib(IM) was determined by MTT method.Meanwhile,the related protein of BCR/ABL fusion protein was detected.Results Compared with the control group,low expression of STAT3 in the K562 cell group using IM treatment had a high inhibition rate,under different concentrations,the inhibition rate was statistically significant(P < 0.05).While in the high expression of STAT3 cells,at the same concentration gradient under the action of IM,and the differences between groups were also statistically significant(P < 0.05).However,there was no statistically significant difference of the inhibition rate in the two control groups.We also detected the similar expression levels of the related protein of BCR/ABL fusion protein,P53,HAUSP and PTEN.Conclusion Low expression of the STAT3 protein enhances the sensitivity of K562 cells to IM,which may result in a further reduction in the subsequent resistance of the K562 cells to IM treatment by inhibiting the STAT3 protein.

OBJECTIVE To observe prenatal nicotine exposure (PNE) induced high sensitivity of hypo?thalamic-pituitary-adrenal (HPA) axis in offspring rats which were fed with a high-fat diet, and to explore the mechanism. METHODS Nicotine (2 mg·kg-1 per day) was injected subcutaneously to preg?nant Wistar rats from gestational day (GD) 9 to GD20,and then the young rats were naturally delivered. After weaning, half of the offspring was fed with a high-fat diet until postnatal weeks (PW) 20. The others were exposed to unpredictable chronic stress (UCS) from PW17 to PW20. The pathological changes in the hippocampus were analyzed by HE staining. The blood concentration of adrenocorticotropic hormone (ACTH) and corticosterone (CORT) was detected by RIA kits and ELISA kits, respectively. Meanwhile, real-time PCR was used to detect the mRNA expression of ACTH releasing hormone (CRH), arginine vasopressin (AVP), vesicular glutamate transporter 2 (VGluT2), glutamic acid decarboxylase 65 (GAD65), mineralocorticoid receptor (MR),glucocorticoid receptor (GR) and glutamic acid decarboxylase 67 (GAD67). RESULTS In the normal control group,UCS treatment increased the level of serum ACTH and CORT 1.96 and 3.24 times in female rats,but 1.63 and 3.54 times in male rats. In the PNE group, UCS treatment increased the level of serum ACTH and CORT 3.96 and 5.98 times in female rats, but 3.04 and 5.22 times male rats. PNE increased the mRNA expression of AVP in the female and male rats 2.04 and 1.13 times in UCS treatment control group, and the mRNA expression of hypothalamus CRH and the ratio of VGLuT2/ GAD65 were increased 2.49 and 1.14 times in female rats, respectively. Furthermore, the nicotine group exhibited histological changes to different degrees in the hippocampus and dentate gyrus area of the hippocampus. In the female and male nicotine groups, the mRNA ratio of hippocampal MR/GR decreased by 88.0% and 86.0% in comparison with the normal control group without UCS, and the mRNA expression of GAD67 was enhanced 1.38 and 1.97 times in female and male rats without UCS. In the female UCS treatment nicotine groups, the mRNA expression of GAD67 was increased 2.17 times compared with the UCS treatment control group. CONCLUSION PNE can induce a high sensibility of HPA axis in offspring rats fed with a high-fat diet. The imbalance of hippo?campus MR/GR and the enhanced expression of GAD67 mRNA may be involved.

OBJETIVO: evaluar la eficacia de la técnica de sedimentación espontánea en tubo - 45 min (TSET) como alternativa a la técnica Ritchie modificada y el examen directo. Diseño Transversal, descriptivo, ecológico y comparativo. Experimento a triple ciego. MATERIAL Y MÉTODOS: Se obtuvo 384 muestras de heces fecales de niños de 7 a 10 años de edad, de tres establecimientos escolares de la ciudad de Cochabamba, escogidas al azar. Se realizó el análisis estadístico con OpenEpi versión 3.01 RESULTADOS: Los parásitos observados con mayor frecuencia entre los Helmintos fueron: Áscaris lumbricoides 142 (36,9%) y Trichuris trichura 89 (23,1%). Entre los Protozoarios: Entamoeba coli 228 (59,3 %) y Iodomoeba butsclii 86 (22,3 %). La técnica TSET detectó poliparasitismo y monoparasitismo de manera similar a la técnica de Ritchie modificada. Los parásitos encontrados con mayor frecuencia en muestras poliparasitarias fueron Entamoeba histolytica y Giardia lamblia, entre los Helmintos dos casos de Ascaris lumbricoides y uno de Himenolepys nana. La técnica propuesta presentó una sensibilidad y valor predictivo negativo 100 %, especificidad 70.5 % y valor predictivo positivo 97.2 %. El coeficiente de concordancia de Kappa entre "Técnica de Sedimentación Espontánea en Tubo -45 min" y la Técnica de Ritchie fue 0,820, (p<0,01); y en relación a las 2 horas de reposo fue Kappa = 1 (p< 0,01). RECOMENDACIONES: Se recomienda utilizar la Técnica de Sedimentación Espontánea en Tubo - 45 min como una prueba alternativa a la Técnica de Ritchie Modificado, por las ventajas que presenta.

The "Spontaneous sedimentation in tube technique - 45 min" (TSET) detected polyparasitism and monoparasitism similar to Modified Ritchie Concentration Technique (MRCT). Polyparasite samples displayed a higher frecuency of Entamoeba histolytica and Giardia lamblia parasites, and among Helminths it was found two cases of Ascaris lumbricoides and one of Himenolepys nana. The proposed technique presented a sensitivity and negative predictive value of 100%, specificity 70.5%, and positive predictive value 97.2%. Kappa concordance coefficient between TSET and Ritchie Technique was 0.820, (p <0.01) and Kappa = 1 (p <0.01) at2 hours of rest. CONCLUSIONS: Using this technique within 45 minutes, as an alternative test to Ritchie Technique, is recommended.

RESUMO Objetivo Verificar a sensibilidade, especificidade e estabelecer pontos de corte para o índice Porcentagem de Consoantes Corretas Revisado (PCC-R) em crianças com e sem transtorno fonológico falantes do Português Brasileiro. Método Participaram 72 crianças com idade entre 5:00 e 7:11 anos, sendo 36 sem queixas de alteração de fala e linguagem e 36 crianças com diagnóstico fonoaudiológico de transtorno fonológico. O índice de gravidade PCC-R foi aplicado nas provas de nomeação de figuras e de imitação de palavras do Teste de Linguagem Infantil ABFW. Os resultados foram analisados estatisticamente. Foi realizada a curva Roc e obtidos os valores de sensibilidade e especificidade do índice. Resultados O grupo de crianças sem transtorno fonológico apresentou valores do PCC-R maiores nas duas provas, independentemente do gênero dos participantes. O valor de corte na prova de nomeação de figuras foi de 93,4%, com sensibilidade de 0,89 e especificidade de 0,94, independentemente da idade. Já na prova de imitação de palavras, os valores obtidos variaram de acordo com a idade. Para a faixa etária ≤6:5 anos, o valor de corte foi de 91,0%, com sensibilidade de 0,77 e especificidade de 0,94. Para a faixa etária >6:5 anos, o valor de corte foi de 93,9%, com sensibilidade de 0,93 e especificidade de 0,94. Conclusão Dada a alta sensibilidade e especificidade do PCC-R, o índice foi efetivo na discriminação e identificação de crianças com e sem transtorno fonológico.


ABSTRACT Purpose The purpose of the study was to determine the sensitivity and specificity, and to establish cutoff points for the severity index Percentage of Consonants Correct - Revised (PCC-R) in Brazilian Portuguese-speaking children with and without speech sound disorders. Methods 72 children between 5:00 and 7:11 years old - 36 children without speech and language complaints and 36 children with speech sound disorders. The PCC-R was applied to the figure naming and word imitation tasks that are part of the ABFW Child Language Test. Results were statistically analyzed. The ROC curve was performed and sensitivity and specificity values ​​of the index were verified. Results The group of children without speech sound disorders presented greater PCC-R values in both tasks, regardless of the gender of the participants. The cutoff value observed for the picture naming task was 93.4%, with a sensitivity value of 0.89 and specificity of 0.94 (age independent). For the word imitation task, results were age-dependent: for age group ≤6:5 years old, the cutoff value was 91.0% (sensitivity of 0.77 and specificity of 0.94) and for age group >6:5 years-old, the cutoff value was 93.9% (sensitivity of 0.93 and specificity of 0.94). Conclusion Given the high sensitivity and specificity of PCC-R, we can conclude that the index was effective in discriminating and identifying children with and without speech sound disorders.
